 SUmmer 2022 CLass Registration Form

    Time:
     10:00 - 3:30
    ​Ages: 5 - 18
    Prerequisites: Must be able to tread water for at least 10 minutes with a life jacket and swim comfortably
    Cost: $300


    Release of liability

    Emergency Treatment Authorization
    I/We the undersigned parent, parents, or legal guardian of this child, a minor, do hereby authorize and consent to any x-ray, examination, anesthetic, medical diagnosis rendered under the general or special supervision of any member of the medical staff licensed under the provisions of the Medicine Practice Act or a dentist licensed under the Dental Practice Act and on the staff of any acute general hospital from the State of Minnesota Department of Public Health. It is understood that this authorization is give in advance of any specific diagnosis, treatment, or hospital care being required but is given to provide authority and power to render care which the aforementioned physician in the exercise of his best judgment may deem advisable. It is understood that effort shall be made to contact the undersigned prior to rendering treatment to the patient, but that any of the above treatment will not be withheld if the undersigned cannot be reached.

    Release
    The undersigned parent/guardian recognizes that an element of risk is involved in all water sports, including sailing. Therefore, to induce the Gull Lake Yacht Club/Sailing School to accept his/her child into the Gull Lake Yacht Club/Sailing School, the undersigned parent/guardian covenants and agrees to hold harmless and indemnify the Gull Lake Yacht Club/Sailing School, its officers, directors, employees, and agents from any and all claims, losses, damages, fees and liability growing out of or in any manner related to injury to a person or damage to any property arising out of or in anywise connected with the operation of the Gull Lake Yacht Club/Sailing School or any activities on or the use of any facilities or equipment of Gull Lake Yacht Club/Sailing School.

    Certification of Swimming Skills
    I/We the undersigned parent, parents, or legal guardian, do hereby certify the student enrolled can swim for 40 yards with a secured PFD and is comfortable in and around the water.

    Parental Agreement
    I/We understand that I/we are responsible for our child’s or grandchild’s behavior and conduct while at the Gull Lake Yacht Club/Sailing School and will see to it that our child adheres to the program rules. I/We agree to assume the obligation for expenses of repair and/or replacement of program equipment that is attributed to our child’s reckless or irresponsible behavior and the expense of medical care if our child is injured. I/We agree to make an appointment for a parent/instructor or coach conference if requested.

    State Mandated Youth Sports Concussion Information
    The state of Minnesota requires that ALL youth sports programs make information accessible to all participating coaches, officials, and youth athletes and their parents or guardians about the nature and risks of concussions, including the effects and risks of continuing to play after receiving a concussion, and the protocols and content, consistent with current medical knowledge from the Centers for Disease Control and Prevention.

    All GLSS staff complete the CDC's 'Heads Up: Concussion in Youth Sports' training for coaches.

    If a concussion is suspected, all GLSS Staff follow the CDC approved 'Heads Up four-step action plan':
    1. Remove participant from activity immediately
    2. Ensure that the participant is evaluated by an appropriate healthcare professional
    3. Inform the participant's parents or guardians
    4. Keep the participant out of play

    Video and Photography Release
    I hereby authorize Gull Lake Yacht Club/Sailing School staff, students, and volunteers to take photography of registered participants for use on behalf of GLSS during GLSS sanctioned camps, classes, volunteer and racing events.

    I hereby authorize GLSS to use any photography/videography of my registered participant on behalf of the GLSS for communication or marketing materials such as, but not limited to: pamphlets, catalogs, GLSS official social media, web development, and advertisements.

    If I do not agree to the 'Video and Photography' release, I may excuse myself/my student from images only by written notification, handed directly to the GLSS Director or onsite office manager on the first day of any registered sessions. A separate written notice is required for each student and registered session.
