    Release of Liability

    Emergency Treatment Authorization
    I/We the undersigned parent, parents, or legal guardian of this child, a minor, do hereby authorize and consent to any
    x-ray, examination, anesthetic, medical diagnosis rendered under the general or special supervision of any member of
    the medical staff licensed under the provisions of the Medicine Practice Act or a dentist licensed under the Dental
    Practice Act and on the staff of any acute general hospital from the State of Minnesota Department of Public Health. It
    is understood that this authorization is give in advance of any specific diagnosis, treatment, or hospital care being
    required but is given to provide authority and power to render care which the aforementioned physician in the exercise
    of his best judgment may deem advisable. It is understood that effort shall be made to contact the undersigned prior to
    rendering treatment to the patient, but that any of the above treatment will not be withheld if the undersigned cannot be
    reached.

    Release
    The undersigned parent/guardian recognizes that an element of risk is involved in all water sports, including sailing.
    Therefore, to induce the Gull Lake Yacht Club/Sailing School to accept his/her child into the Gull Lake Yacht
    Club/Sailing School, the undersigned parent/guardian covenants and agrees to hold harmless and indemnify the Gull
    Lake Yacht Club/Sailing School, its officers, directors, employees, and agents from any and all claims, losses,
    damages, fees and liability growing out of or in any manner related to injury to a person or damage to any property
    arising out of or in anywise connected with the operation of the Gull Lake Yacht Club/Sailing School or any activities
    on or the use of any facilities or equipment of Gull Lake Yacht Club/Sailing School.

    Certification of Swimming Skills
    I/We the undersigned parent, parents, or legal guardian, do hereby certify the student enrolled can swim for 40 yards
    with a secured PFD and is comfortable in and around the water.

    Parental Agreement
    I/We understand that I/we are responsible for our child’s or grandchild’s behavior and conduct while at the Gull Lake
    Yacht Club/Sailing School and will see to it that our child adheres to the program rules. I/We agree to assume the
    obligation for expenses of repair and/or replacement of program equipment that is attributed to our child’s reckless or
    irresponsible behavior and the expense of medical care if our child is injured. I/We agree to make an appointment for a
    parent/instructor or coach conference if requested.

    State Mandated Youth Sports Concussion Information
    The state of Minnesota requires that ALL youth sports programs:
    'make information accessible to all participating coaches, officials, and youth athletes and their parents or guardians
    about the nature and risks of concussions, including the effects and risks of continuing to play after receiving a
    concussion, and the protocols and content, consistent with current medical knowledge from the Centers for Disease
    Control and Prevention.

    All GLSS staff complete the CDC's 'Heads Up: Concussion in Youth Sports' training for coaches.
    If a concussion is suspected, all GLSS Staff follow the CDC approved 'Heads Up four-step action plan':
    1. Remove participant from activity immediately
    2. Ensure that the participant is evaluated by an appropriate healthcare professional
    3. Inform the participant's parents or guardians
    4. Keep the participant out of play

    Video and Photography Release
    I hereby authorize Gull Lake Yacht Club/Sailing School staff, students, and volunteers to take photography of
    registered participants for use on behalf of GLSS during GLSS sanctioned camps, classes, volunteer and racing events.
    I hereby authorize GLSS to use any photography/videography of my registered participant on behalf of the GLSS for
    communication or marketing materials such as, but not limited to: pamphlets, catalogs, GLSS official social media,
    web development, and advertisements.

    ​If I do not agree to the 'Video and Photography' release, I may excuse myself/my student from images only by written
    notification, handed directly to the GLSS Director or onsite office manager on the first day of any registered sessions.
    A separate written notice is required for each student and registered session.

    Gull Lake Sailing School COVID-19 Safety Policy

    Effective May 27, 2020

    This policy applies to all sailors, parents and instructors. As always, the safety and well-being of our sailors is our highest priority. This year, COVID-19 presents some challenges, but the Gull Lake Sailing School (GLSS) is committed to striving for a safe environment that enables all sailors to continue to grow their knowledge and love of sailing. As such, the GLSS staff and board of directors have carefully laid out plans for this summer and fall high school season.

    To accomplish the safety and well-being of all sailors, their families and the GLSS instructors, it is critical that each sailor and their parents understand and follow this policy. Instructors will be working with sailors to help them understand and follow their responsibilities for creating a safe environment for themselves and their peers. Sailors that disregard their responsibilities will be asked to leave the event with their money refunded on a prorated basis.

    GLSS has redesigned all its activities with a focus on structured social distancing, hand hygiene and facial protection. Six-foot radius seating arrangements have been developed for instructional approaches to accommodate spread-out chalk talks. Buffs or similar facial protection will be required for indoor common spaces and anytime a sailor is within a six-foot radius of another person. Thermometers will be available to assist instructors in their immediate response to a sailor that isn’t feeling well. Finally, GLSS instructors have received additional training on CDC and Minnesota Department of Health guidelines as well LMSS COVID-safe coaching requirements.
    With all the above mitigations in place, each sailor, parent and instructor play a critical role.

    SAILOR RESPONSIBILITIES AND EXPECTATIONS
    • Do not attend sailing events if you have any degree of fever, are feeling sick or have had contact with someone with COVID-19 in the last 14 days. If you begin to feel ill, let your instructor know right away.
    • Always maintain a six-foot (at least two arm’s length) radius from your fellow sailors and instructors. Be respectful of other sailors’ space and help them remember yours. This includes while on the docks.
    • Stick to your own boat, please do not help others with their boats or equipment. This will eliminate contamination between sailors and boats. Note: GLSS will be thoroughly washing and sanitizing every boat between sessions. Additional cleaning will be done if necessary
    • Wash or sanitize your hands when you arrive at GLSS and after you are done de-rigging your boat. Hand sanitizer always goes on after taking off sailing gloves and before sunscreen is applied or reapplied.
    • You will be expected to use a buff or other facial protection and wear it over your nose and mouth anytime you are inside a building or within a six-foot radius of another person. And are encouraged to wear it for the duration of class.
    • Don’t share your water bottle, food or sunscreen with another sailor. While it is nice to share with a friend, we want to keep potential germs to ourselves.
    • No guests on your boat. Don’t be a guest on another sailor’s boat. Practice social distancing at all times including when on the water.
    PARENT RESPONSIBILITIES AND EXPECTATIONS
    • Make sure your child understands the importance of following the above responsibilities and expectations
    • Help your child understand that social distancing, hand sanitizing and their facial protection are a critical part of safe sailing this summer (much like the importance of sunscreen and their life jacket)
    • Do not send your child to any activity if they have cold or flu symptoms or if a member of the family has cold or flu symptoms. Please err on the side of caution and notify the GLSS Director (Phone number below and on the GLSS Website). Missed sailing school days due to sailor or family member illness will be reimbursed on a prorated basis.
    • Contact the GLSS Director if your child develops symptoms within three days after attending an activity. Your diligence in keeping the GLSS Director informed is key to a safe summer and fall for all.
    • Don’t hesitate to reach out to the GLSS Director or an instructor with any questions or concerns
    INSTRUCTOR RESPONSIBILITIES AND EXPECTATIONS
    • Lead by example and set the tone for what “good” looks like. Make sure each sailor understands their responsibilities as defined above including expectations for safe distancing, hand hygiene and wearing facial protection. Observe each sailor throughout the day and provide timely feedback on how they are doing.
    • Be approachable and get to know your sailors. Make it easy for them to share their concerns and comfortable to tell you if they are not feeling well.
    • Ensure all instructional, transitional and downtime activities promote healthy social distancing practices
    • Allow extra time for transitional activities (e.g. rigging/derigging) to help avoid crowing on docks
    • Provide additional boat sanitizing using a chemical disinfect at the beginning or the end of each week to supplement cleaning performed by sailors
    • Follow GLSS coach requirements for personal illness reporting (see separate document)
    Each party agrees that this Agreement may be electronically signed, and that any electronic signatures appearing on this Agreement or such other documents are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
